Why one document, not two? California's statutory Advance Health Care Directive replaces the older separate “Healthcare Power of Attorney” and “Living Will” forms. One signed AHCD appoints your health-care agent and records your end-of-life preferences in a single instrument that hospitals and physicians are trained to accept. Together with your HIPAA Authorization, these two documents form a complete health-care decision-making package.
California Deed

Transfers title to your real property, including into a Living Trust when appropriate.
What you get:
One completed California Grant Deed, pre-filled with your grantor, grantee, property address, APN, and legal description (leave APN or legal description blank if you do not have them yet — copy them from your current deed before signing).
A pre-filled official county PCOR (BOE-502-A) when your county is supported. If an official fillable form is not available for that county, the packet includes a pre-filled PCOR worksheet you can copy onto the county form.
A signing and recording instruction page that tells you where to sign, what the notary needs, and how to submit the deed at your County Recorder.
What you do next: Sign the deed in front of a California notary public. Sign and date the last page of the PCOR (no notary required for the PCOR). Submit the notarized deed together with the PCOR at your County Recorder’s office — submitting the PCOR with the deed avoids the non-PCOR filing penalty.
